State fines Earthworks, three other companies
Four Hawai'i companies have been fined between $2,300 and $6,500 by the state Health Department's Clean Air Branch, it was announced yesterday.
The companies are Earthworks Pacific Inc., Goodfellow Brothers Inc., Tesoro Hawai'i Corp. and Meadow Gold Dairies Inc.
Earthworks Pacific was fined $6,500 for not taking reasonable precautions to prevent visible fugitive dust from becoming airborne at the Kaua'i Marriott Resort and Beach Club construction site at 3610 Rice St. in Lihu'e on Feb. 29 and March 31. The DOH and Earthworks Pacific will enter into a consent order on the issue in the near future.
The other three companies paid their fines.
Goodfellow Brothers paid a $2,400 fine for late submittal of its semi-annual report for January through June 2007 as required by its permit. The violation was discovered on Jan. 30 of this year during a records review of the 780 TPH stone processing plant at 480C Welakahao Road in Kihei, Maui.
Tesoro Hawai'i paid a $2,300 fine for failing to perform a sulfur dioxide test, specifically a relative accuracy test audit on its sulfur recovery unit's tail gas incinerator sulfur dioxide continuous emissions monitoring system in the third quarter of 2007. The violation was discovered during a records review Feb. 8 of the oil refinery at 91-325 Komohana St. in Kapolei.
Meadow Gold Dairies paid a $2,400 fine for late submittal of its semi-annual report for July through December 2007 as required by its permit. The violation was discovered March 28 during a records review of the plant's two 150-horsepower Cleaver-Brooks boilers at 910 Sheridan St.
